URGENT ADVISORY: The Form 27-B Recall Incident. Pursuant to internal audit reference #2025-1847, it has been determined that Form 27-B revisions published between October 3 and October 17 contained an error in field 412 (Procedural Amendment Authorization Code). The auto-populated code referenced Regulation 312-A instead of Regulation 312-A-revised. All 2,300 forms submitted during this period are hereby invalidated. I discovered the error at 2:47 AM on a Tuesday. I have not slept well since. All affected parties must re-submit.
